Abstract:

زمینه: حساسیّت سیستم‌ های مغزی/رفتاری نشان ‌دهنده‌ ی آمادگی ابتلا به اشکال متعدد آسیب‌ ‌روانـی از جمله اختلال‌ های اضطرابی است، از سوی دیگر، عدم تحمل بلاتکلیفی نیز به عنوان یک سازه‌ی فراتشخیصی تقریباً در همه‌ ی اختلال‌ های اضطرابی، ازجمله اختلا اضطراب بیماری وجود دارد. با این حال، روابط میان این متغیرها در قالب یک مدل منسجم و در جمعیت بالینی نیاز به بررسی بیشتری دارد. هدف: پژوهش حاضر با هدف بررسی نقش واسطه ‌ای عدم تحمل بلاتکلیفی در تبیین رابطه بین سیستم‌های مغزی/رفتاری با اختلال اضطراب بیماری انجام شد. فرض بر این است که سیستم ‌های مغزی/رفتاری و عدم تحمل بلاتکلیفی در تعامل با یکدیگر به اختلال اضطراب بیماری منجر می‌شوند. ولی با وجود اهیمت این مدل، پژوهش‌های کافی در این راستا انجام نشده است. روش: روش اجرای این پژوهش توصیفی، همبستگی و از نوع معادلات ‌ساختاری بود. جامعه مورد نظر بالینی بود که شامل تمامی درمانجویان دارای اختلال اضطراب بیماری (زن و مرد) مراجعه ‌کننده به مراکز روانشناختی و روان‌ پزشکی شهرستان ارومیه (افراد 17-40 ساله) در سال ‌های 1399-1400 بود. برای این منظور 200 نفر از درمانجویان با روش نمونه ‌گیری هدفمند (با تشخیص روان ‌شناس یا روان‌ پزشک) انتخاب شده و به پرسشنامه‌ های اضطراب سلامتی (سالکوویسکیس و همکاران،2002)، مقیاس عدم تحمل بلاتکلیفی (فریستون و همکاران، 1994) و مقیاس سیستم‌ های بازداری/فعال‌ ساز رفتاری (کارور و وایت، 1994) پاسخ دادند. داده ‌ها با استفاده از روش آماری مدل‌یابی معادلات ساختاری مورد تجزیه و تحلیل قرار گرفتند. یافته‌ ها: یافته ‌ها حاکی از آن است که سیستم بازداری رفتار با ضرایب اثر 0/340 و 0/519 و سیستم فعال سازی رفتار با ضرایب اثر 0/219- و 0/233- به ترتیب بر اضطراب سلامتی و عدم تحمل بلاتکلیفی تأثیر مثبت و منفی دارند. همچنین عدم بلاتکلیفی با ضریب اثر 0/414 بر اضطراب سلامتی تأثیر مستقیم و مثبتی دارد. نتیجه‌گیری: یافته‌های ما نشان می‌دهد که عدم تحمل بلاتکلیفی و سیستم‌ های مغزی/رفتاری می‌ توانند از جمله سازه‌ های زیربنایی در اختلال اضطراب بیماری باشند. می‌ توان نتیجه‌ گیری کرد که فعالیت سیستم ‌های مغزی/رفتاری و عدم تحمل بلاتکلیفی مکانیسم ‌هایی اثرگذار در تداوم اضطراب بیماری هستند و در نظر گرفتن این روابط می ‌تواند در سبب ‌ شناسی و درمان افراد مبتلا به اختلال اضطراب بیماری سودمند باشد.

Background: The Behavioral Inhibition System (BIS) and Behavioral Activation System (BAS), which primarily underlie emotions and behaviors, are associated with Illness Anxiety Disorder. However, the reasons behind these associations require further exploration. Aims: The aim of this study was to investigate the mediating role Intolerance of Uncertainty (IU) in explaining the relationship between Brain / Behavioral Systems and Illness Anxiety Disorder (IAD). It is hypothesized that Brain / Behavioral Systems and Intolerance of Uncertainty interacting with each other lead to IAD. However, despite the importance of this model, not enough research has been done in this regard. Methods: The method of this research was descriptive (non-experimental) and the research design was correlational and structural equations. The target population was clinical and it included all patients (male and female) referred to psychological and psychiatric centers in Urmia city. The sample included 200 anxious clients which were selected by purposive sampling method (diagnosed by a psychologist or psychiatrist) and responded Short Health Anxiety Inventory (SHAI), Intolerance of Uncertainty Scale (IUS) and Behavioral Approach System and Behavioral Inhibition System Scales (BAS / BIS). Results: Findings indicate that BIS with effect coefficients of 0.340 and 0.519 and the BAS with effect coefficients of -0.219 and -0.233, respectively, have a positive and negative effect on IAD and IU. Also, IU with a coefficient of effect of 0.414 has a direct and positive effect on IAD. The t-statistic for indirect path coefficients is greater than 1.96 and the significance level is less than 0.05, which indicates the significance and confirmation of all indirect coefficients in the research model (t≥1.96; p≤0.05). Precisely, the findings indicate that the BIS and BAS with indirect effect coefficients of 0.215 and -0.097 due to mediation of IU have a positive and negative effect on IAD, respectively. Conclusion: It can be concluded that the activity of Brain /Behavioral Systems and IU are effective mechanisms in the persistence of IAD and considering these relationships might be beneficial in the etiology and treatment of people with IAD.
