چکیده:
سابقه و هدف: فرهنگ ایمنی بیمار یکی از عناصر اصلی ارتقای ایمنی و بهبود کیفیت مراقبت از بیمار می باشد. مطالعات قلیلی در خصوص وضعیت فرهنگ ایمنی بیمار در کشور انجام شده است. در این مطالعه با استفاده از پرسشنامه SAQ دانشگاه تگزاس، وضعیت فرهنگ ایمنی بیمار در بیمارستانهای آموزشی دانشگاه علوم پزشکی شهید بهشتی مورد بررسی قرار گرفت.
مواد و روشها: در یک مطالعه مقطعی در یک بازه زمانی 4 ماهه در سال 1390، از میان 9 بیمارستان آموزشی دانشگاه علوم پزشکی شهید بهشتی به شکل هدفمند و با توجه به بزرگی و پراکندگی جغرافیایی، 4 بیمارستان (دو بیمارستان بزرگ بیش از 300 تخت و دو بیمارستان کوچک کمتر از 300 تخت) از مناطق شرق، شمال و غرب تهران انتخاب شدند. جامعه پژوهش این مطالعه کلیه داروسازان، دستیاران پزشکی، پرستاران، تکنسین های اتاق عمل، کارشناسان تغذیه، و شاغلین در بخشهای رادیولوژی و آزمایشگاه بودند. ابزار گردآوری داده ها در این مطالعه پرسشنامه معتبر و پایای ترجمه شده (88/0 r=) دانشگاه تگزاس امریکا بود که از طریق 8 حیطه، وضعیت فرهنگ ایمنی بیمار را در بیمارستانها مورد سنجش قرار می داد.
یافته ها: 58% از افراد شرکت کننده در مطالعه، پرستاران بودند و بیشتر از 2 سال سابقه کار در بیمارستان داشتند. سطح فرهنگ ایمنی بیمار در بیمارستانهای مختلف متوسط بود و بین بیمارستانها تفاوت آماری معنی داری دیده نشد. بیمارستانها از نظر حیطه اقدامات و انتظارات مدیریت در راستای بهبود ایمنی بیمار و حیطه مسائل مربوط به کارکنان اختلاف معنی داری با هم داشتند (009/0>(p. پست سازمانی افراد تفاوت معنی داری در نمره کلی فرهنگ ایمنی و پنج حیطه از حیطه های فرهنگ ایمنی در بین بیمارستانها ایجاد نمود (05/0>(p. قویترین بعد فرهنگ ایمنی بیمار در بیمارستانهای مورد مطالعه، حیطه کار تیمی با 74% پاسخ مثبت بود.
نتیجه گیری: فرهنگ ایمنی بیمار حاکم بر بیمارستانهای مورد مطالعه در سطح متوسط بود. از این رو، در فرهنگ موجود حاکم بر بیمارستانها، طراحی مداخلات مربوط به بهبود فرهنگ با توجه به ویژگیهای هر بیمارستان مورد نیاز است.
Background and Aim: Patients’ safety culture is one of the key elements to safety promotion and improvement of the quality of patient care. Few studies have been conducted about patients’ safety culture in the country. Here، patients’ safety culture status among educational hospitals of Shahid Beheshti University of Medical Sciences was studied using Texas university SAQ questionnaire.
Materials and Methods: In a cross sectional study in 2011، four out of nine hospitals affiliated with Shahid Beheshti University of Medical Sciences were chosen from North، East and West of Tehran metropolitan، based on their magnitude and geographical dispersion. Two of these hospitals were big (more than 300 beds)، and two were small (less than 300 beds). The study population consisted of pharmacists، medical assistants، nurses، operating room technicians، nutrition experts، and practitioners in radiology and laboratory departments. In this study data collection tool was a validated & reliable (r=0.88) Farsi translated version of Texas university questionnaire (SAQ)، focusing on eight areas of patients’ safety culture.
Results: 58% of samples were nurses with more than two year of experience. In this study، patients’ safety culture was in the moderate level among studied hospitals and no significant difference was observed between them. However، a significant difference was seen among hospitals، in terms of management and expectations of safety improvement along with staffs concerns (p<0.009). A significant difference was also seen in total score of safety culture and five areas of safety culture، based on organizational position of the respondents (p<0.05). The strongest dimension of patients'' safety culture in studied hospitals was team work area with 74% positive responses.
Conclusion: Patients'' safety culture in the target hospitals was in the middle level. Hence، interventions for cultural improvement considering hospital characteristics are required.
